My gear knob has scratches on so I was thinking about getting a new one, my question is, are they all the same?

I was looking at some toyota ones (see here) and wanted to double check they would fit, I presume they would? I liek a lot of the Momo ones too

The other question is would sideskirts made for the IS300 fit the IS200? I know there are problems with the rear bumpers but as far as I know The sides should be ok. Right? or wrong?

There should be no problem with the side skirts.

The bumper problem is not between the IS200 and the IS300, it's between US and Euro cars. So a bumper designed to fit an IS200 will fit a euro IS300 without any problems.
