Cd Changer Fault

Might be worth having a look at this:

Sounds like a problem with the actual changer in that a disk is stuck or not sat properly.

A few things I've seen on the net say either powering off the entire car by removing the key should temporarily fix it, if not someone else managed to get past it by removing the connections to the stock radio, leaving it a while and then reconnecting it all up.

As a matter of interest are you using copied CDs in there as CDRs can cause issues with the Lexus units?

If you try the following on Google you'll get quite a few results on this error:

lexus rx300 "error 4" +site:lexusownersclub.com

lexus rx300 "error 4" +site:clublexus.com

Doesn't sound good though to be fair, could be new headunit time :(
