i had this same issue with my 2007 GS450h, sounded like a Vauxhall Astra tappets noise 😞 was gonna get it check but then it died while driving luckily i work for a Toyota dealership and they looked into it for me. changed the high pressure fuel pump as fuel pressure was low but didn't work, then changed the in tank fuel pump and filters and problem was fixed and the tapping was gone. been working like a dream again and i have my baby back 🙂

I have a Lexus GS450H I was driving and I had loss of engine power, managed to get it off the road with hybrid power. AA came and read the ecu with error codes P3190 + P3191. I work for Toyota so had the techs plug it in and it they see that the fuel pressure is low, so I changed the high pressure fuel pump. took it for a drive and it felt like it was holding back after I got to about 70MPH so was still unclear what the issue is? put some fuel system cleaner in incase it needed a clear through and went to drive it home but then the same original Issue occurred and no engine power and only hybrid power. wonder if anyone can shed some light as I don't wanna spend £100s on new parts when they might not solve the issue.
